Compass

As a CHTN subscriber, access to our Compass tool is available at no additional cost. Compass tracks 33 elements of outpatient clinical care that are important to the health of patients.

Measures are grouped in the following clinical bundles:

• Preventive Care – disease prevention and early disease detection measures
• Chronic Disease Care – focused on chronic diseases prevalent in our patient population

Data is captured in the Compass Registry through entry into the EPIC patient EHR. Compass allows providers to review individual patient information, compliance with each measure for all patients in their practice, and overall compliance using data updated daily. At the time of a visit, patient data is embedded and available on all outpatient clinical measures.
