Walk-in Sports Physicals

Presented by Henry Ford Pediatrics

Henry Ford Pediatrics is excited to conduct sports physicals for students planning to participate in athletics during the 2024–2025 school year. All walk-in locations are open to students ages 11-18 from any school district needing a sports physical. No appointment is necessary.

Students must bring:

• Sports Physical
• Cash payment of $20.00. Cash or check only. Credit cards will not be accepted.
• Payment is accepted at the time of the sports physical. Please note, athletes who are not accompanied by parents can only be seen by the physician if all forms are completely signed.

Please print this form and have it completed before attending the sports physical. The physicals cannot be conducted without it.
