Studies have shown that patients who use blood-thinning (anticoagulant) medications such as warfarin (Coumadin®) experience fewer complications, hospital admissions and emergency department visits when they work with a specially trained professional. The pharmacists and pharmacy technicians at Henry Ford Allegiance Medication Therapy Management Clinic have the expertise and training to help you manage your blood-thinning medications safely and effectively.
Our team will connect with your referring doctor to make sure you are taking the correct dose and that your anticoagulant safely interacts with your other medications. Your first visit will take up to one hour, so the pharmacist can review your medical history, provide information and answer all your questions. Follow-up visits, if needed, usually take 15 minutes and will include a finger-stick blood test to measure the effectiveness of your medication. Periodic laboratory tests will also be scheduled to keep you on track.
