PACE Southeast Michigan
PACE Southeast Michigan provides an alternative to nursing home care for frail, elderly people. By bringing together all the care needed, participants are able to remain independent in their own homes and communities. Patients and their caregivers also are provided respite care. The comprehensive care, both medical and social are provided at our offices and the elder's home, as well as in the hospital or nursing home, when necessary.
Our services
Elderly people who choose to enroll in the program receive all medical, specialty and in-home services as arranged by our staff. At the time of initial assessment, the team develops a care plan for each participant. Depending on individual needs, participants come to our location one or more days per week, where they receive medical and personal care. Recreational and therapeutic activities take place through the day and meals are provided, as well as routine specialty care such as foot care, dentistry and hearing services. Prescription drugs and home delivered meals also are provided. Transportation is provided to and from our location and to specialized appointments. Our staff includes:
